Registration & consent form

This consent form is for under 18s who wish to attend our Body Positive Life Drawing Classes. This form must be completed by a parent/guardian before students are able to participate in any activities. Failure to return a completed form means you will be unable to attend our sessions.

Parents/carers must accompany these students to the sessions and students must stay with the teacher, Kate, for the duration. Parents/carers can stay on campus while the sessions take place, or join in with the creativity!

All personal information we receive is stored securely. We do not keep or have access to your card details.

I DO/ DO NOT consent to my child taking part in the life drawing class under the knowledge that models will be posing nude (all models are DBS checked and accompanied by a teacher).

I DO / DO NOT give consent for Our Creative Community staff to consent to medical or surgical treatment for my child on my behalf, should it be necessary.

I acknowledge that Our Creative Community staff DO NOT have responsibility for my child prior to arrival or following departure from the activity sessions. Student drop off/collection will be by X block (details on Life Drawing page). Please be prompt with drop off/collection.
