Registration & consent form

This consent form covers our Young Architect Group classes. This form should be completed by a parent/guardian if you are under 18, before you are able to participate in any activities. Failure to return a completed form means you will be unable to attend our sessions.

Once we receive the below registration form, we will be in touch to discuss start dates (space dependent). After a free taster session, registration is termly and payment is made through online invoices (installment options are available).

We do offer free community spaces. Please include this request in the below form if you wish to enquire under 'additional information'.

All personal information we receive is stored securely. We do not keep or have access to your card details.

I acknowledge that Our Creative Community staff DO NOT have responsibility for my child prior to arrival or following departure from the activity sessions. Student drop off/collection will be by X block (details on Young Architects Group page). Please be prompt with drop off/collection.
